The formal head and shoulders portrait is taken for awards, biographies, command walls and more.

Formal head and shoulders only require the top half of the service dress uniform with required name tag, correct rank, ribbons and badges. Please reference AFI 36-2903 for assistance.

The Public Affairs Office has the right to decline service to the member if uniform is wrinkled, dirty or not presentable.
